Primary Care and Prevention Focused Care Model (PCPFCM)
Your primary care provider (PCP) is your number one source for medical care, but your PCP can also connect you to support for things such as food, housing, and counseling.
Your PCP will ask you questions during your visit to find out if you could use extra assistance. These questions are called a Health Risk Assessment (HRA).

A wellness visit is preventive care that you should get every year. You should go to this doctor visit even though you feel fine. Your health is important to many areas of your life. You and your PCP work together to help maintain or improve your health and help prevent problems that may hurt your health in the future.
Your PCP will let you know if you are due for any cancer screenings such as a mammogram, pap test, or colonoscopy.
Your PCP will let you know if you are due for any vaccines, such as a tetanus booster or hepatitis vaccine.
At a wellness visit, your PCP may listen to your heart or lungs, and look in your ears, eyes, or throat. They may gently press on your stomach area or lower abdomen.
Your PCP will ask you questions about your past or current health conditions and any medications you are taking (including over-the-counter medications like Tylenol, Advil, vitamins, or herbal supplements).
Your PCP may ask other questions to find out if you feel like you have enough food to eat, and if you have a safe place to stay and your housing is stable. They may ask if you need help with everyday tasks like getting dressed, taking a shower, making meals, or going grocery shopping. Knowing the answers to these questions helps your doctor provide you with the best care for your needs.

The HRA is a series of questions that your provider will ask you during your routine wellness visit. The questions will help your provider assess your physical health, emotional health, whether you could benefit from resources to help with food, housing, coaching to help you manage a medical condition, and more.
Your provider may have their own set of questions for you. Or, you may print this HRA (PDF) and complete it and bring it with you to your appointment.

You may change your PCP to the provider you are seeing as long as that provider is in our network.

If you have a different in-network doctor you would like to see, you may change your PCP at any time.
If you need to find a new PCP, you can search for a new provider or you can call Member Services at 1-833-704-1177 (TTY 1-855-534-6730).
Your primary care provider (PCP) can be a doctor, nurse practitioner, physician assistant, or another type of provider who will care for your health and help you get referrals for specialized services if you need them.
Women can choose an OB/GYN to serve as their PCP. Women do not need a PCP referral to see an OB/GYN doctor in our network or another provider who offers women's health care services. Women can get routine checkups, follow-up care if needed, and regular care during pregnancy.
If you have a complex health condition or a special health care need, you may be able to choose a specialist to act as your PCP.

Did you know? If you have a family member or friend who can drive you to your appointment, they can receive mileage reimbursement. You still need to call the number below two business days prior to the visit to be able to receive reimbursement.
