Special Health Programs

AmeriHealth Caritas New Hampshire has special programs to help you get and stay healthy. You do not need a referral for any of these programs.

Care management

AmeriHealth Caritas New Hampshire has care management programs for members who have special health care needs. These can be behavioral health conditions or chronic conditions. Examples are heart disease or high blood pressure. These programs can help you learn more about your condition. They can help you manage your condition. They may help improve your quality of life.

A Care Manager will manage your care. They will send you information about your condition. Your Care Manager will also call you to find out how you are doing. They will help you to get program services.

You can sign up for a program in the following ways.

  • Log in to the member portal and go to Enroll in a Special Program.
  • Call Member Services at 1-833-704-1177 (TTY 1-855-534-6730) and ask to speak with a Care Manager. 
  • Your health care provider may talk to you about joining a program. They can call us to help enroll you.
  • Based on your health history, we may see that a program could help you. We will send you information in the mail about how to enroll in the program.

If you are enrolled in one of the programs and no longer want to be part of it, do one of the following:

  • Call Member Services at 1-833-704-1177 (TTY 1-855-534-6730) and ask to speak with a Care Manager.
  • Write to us at the address below.

Leaving a program will not change your benefits. It will also not change the way AmeriHealth Caritas New Hampshire or the New Hampshire Department of Health and Human Services treats you.

Complex care management

AmeriHealth Caritas New Hampshire has integrated care management programs for members who have special health care needs, behavioral health conditions, and chronic conditions such as asthma, diabetes, and high blood pressure. In this program, you will work with a Care Manager who will help you get the services and information you need to improve your health.

Services include helping you to:

  • Get a complete health evaluation with you and your caregiver.
  • Find information about your medical needs.
  • Get your medicines and medically necessary equipment and supplies.
  • Arrange for transportation.
  • Find specialists.
  • Connect to community resources.
